Can I question your definition of compulsory euthanasia? If something is compulsory there is no element of choice. My understanding of this and from teaching AQA morality is that an example of compulsory euthanasia would be that similar to the Nazi euthanasia of Jews, the disabled, the sick, the mentally impaired. Non- Voluntary euthanasia is, as in the case of Tony Bland, where the decision is made for them in their best interests. All three, Voluntary, Non- voluntary and compulsory, can be passive or active depending on the methods used. (Sorry, I'm not trying to rant just thought I should say something!)

Thank you for sharing such a thorough exploration of the topic. Just one thing - the Tony Bland case was a landmark because the decision was made to withdraw his food. In other words, he was starved to death which was very contraversial. Is food treatment? It may have been the gateway to the current 'Liverpool Pathway' which has its own controversies!
