I have used the notes at the back of each Phase in L&S to make a secure judgement sheet for Phases 2 - 5. I wouldn't necessarily use one for each pupil but find them really useful for anyone I am unsure about or for my lower ability children to pick up on any gaps. The words to blend and segmen More…t are just examples and I pick different words to spell than those just read. Hope someone finds them useful! Sorry just noticed Phase 5 is currupt, will pick it up from school network and then try to reload. Done!

Really useful assessment, thank you for collating it all together. Just to let you know the phase 5 doc is corrupt - is it possible for you to reload it?
