Teacher and pupil behaviour evaluation sheet

This is a very simple chart for teachers to fill in the behaviour of the child for each lesson. Where it differs from the usual one is that there are smiley faces etc for the child to evaluate their own behaviour. This proved to be a lot more useful than making children write comments. I teach in a school for MLD children and it did work much better for some children, which is why I am sharing it. Obviously you can alter it to fit your day. It only covers three days which was a useful length for our children. It made a useful discussion document, and progress could been seen more clearly.
