This is a certificate I created for my 'garment manufacture' lesson. I print off a couple each week, then throughout the lesson I look for super star students that do something nice, or have completed a task correctly.
I then give out certificates to those I feel deserve one at the end of each lesson, at the end of each term we count which student has the most certificates and they win a small prize (chocolate, small sewing pack, something crafty).
As I teach in FE, with my adult groups attendance and punctuality can be quite tricky, however since implementing the reward system, student participation, general respect, class tidiness, attendance and kindness within the group has definitely been boosted!
You could use for every lesson, or change the wording so that a student wins one per end of term.
