We developed this form for when we are away from preschool on trips, doing Forest School and we keep it in our emergency grab bag as well as our first aid kit.
We use it to monitor any casualty we might need to keep a close eye on or if we are waiting for an ambulance. We also have it in case we ever have a few incidents so we don’t confuse them.

It is a useful document to pass onto the ambulance driver, hospital or parent to ensure they have the correct information.

Thankfully we have never had to use it (I am touching wood) but it’s a ‘peace of mind’ document.
